Implementing failing safely

During the past two years, Port Otago has moved to the Failing Safely model.

Failing Safely focuses on workplace engagement and the reality that mistakes do happen.

Port Otago Head of Safety Ross Buchan says the vision is to become an error tolerant workplace. “That means that, when we fail, we ensure we ‘fail safely’ and protect kaimahi from critical risk or fatal events.”

Ross says the new approach has been received well by the team. “It feels logical and realistic to them and gives them confidence that, when things go wrong, the focus is on learning and improving.”

Over 2024/25, two important improvements were made:

  1. Safe standing zones for road interchanges
  2. Packing pod areas
